FEES, INSURANCE & PAYMENT POLICY

If you are visiting our office for the first time, please bring all necessary information regarding your insurance and any medication you are currently taking. We will be glad to file your insurance or help you properly submit your claims. In most instances, patients will be required to pay insurance deductibles or co-payments at the time of an office visit when services and related tests are rendered. To keep fees to a minimum, we would request payments be rendered at the time of service.

Our practice participates in many insurance plans, Health Maintenance Organizations (HMOs) and Preferred Provider Organizations (PPOs). We also accept Medicare, Medicaid, and Blue Cross/Blue Shield. Please feel free to ask our receptionist about your particular insurance plan.
